Everything you need to know about hair transplantation
What is the hair transplantation?
Hair transplantation involves extracting hair follicles from the donor area, typically the back of the head, and implanting them into the recipient area.
How many methods are available for hair transplantation?
There are 3 different methods:
1️⃣ FUE (Follicular Unit Extraction)
2️⃣ Sapphire
3️⃣ DHI (Direct Hair Implantation)
There is another less commonly used method named FUT (Follicular Unit Transplantation).
Due to its invasiveness and the resulting linear scar at the donor site, this method is not used in Turkey anymore.
FUT involves surgically removing a strip of skin from the donor area, typically the back of the head, and then dissecting it into individual grafts for transplantation.
The scar may be noticeable, especially if the patient wears short hair.
What are the differences between hair transplantation methods?
The FUE and sapphire methods are the same and they involve 3 steps:
1️⃣Harvesting the hair grafts from the donor area.
2️⃣Creating tiny channels using extra tiny blades in the recipient area.
3️⃣Implanting the harvested grafts into the channels.
In standard FUE, the blades are made of steel, but in the Sapphire method, the blades are made of sapphire, resulting in more precise incisions, a faster healing period, and potentially less postoperative discomfort.
DHI is the most advanced method in which grafts are implanted directly into the recipient area without the need for incisions or creating channels using a specialized tool called the Choi Pen.
What are the advantages of each method of hair transplantation?
Advantages of the FUE and Sapphire methods:
These methods are well-suited for a limited number of grafts and a larger recipient area. Surgeons will be able to strategically position grafts, enhancing the natural appearance and optimizing coverage.
Advantages of the DHI method:
The DHI method offers more precise and controlled placement of grafts, providing greater control over the angle of hair growth. It also facilitates faster healing, ensures a higher graft survival rate, delivers more natural-looking results, eliminates the need for shaving hairs, involves minimal handling of grafts, and does not require shaving the hair.
What is a graft?
A graft is a piece of tissue containing hair follicles that is transplanted from one area of the body (typically the back of the scalp, known as the donor area) to another area (the recipient area) where hair loss or thinning has occurred.
The graft contains one or more hair follicles along with surrounding tissue and is harvested and transplanted to restore hair in areas of baldness or thinning.
Which method of hair transplantation is more suitable for you?
Determining the most suitable method of hair transplantation depends on various factors such as your hair loss pattern, the extent of hair loss, the quality of your donor hair, your scalp condition, your desired outcome, and your preferences.
However, in general, if your donor area has good density and the recipient area is small, and you are seeking optimal density, the DHI method may be better for you.
This method allows for precise and controlled placement of grafts, ensuring a natural appearance and efficient coverage.
On the other hand, if you have a weak donor area with a large recipient area and you desire even spread of hair over the entire bald area, the FUE and Sapphire methods may be more suitable. These methods offer the flexibility to strategically position grafts, optimizing coverage and achieving desired density.
It’s important to note that if you already have some grafts in the recipient area that you wish to preserve during hair transplantation, the DHI method is advised. This is because with FUE or Sapphire methods, creating incisions near existing grafts could potentially harm them. DHI’s direct implantation technique minimizes the risk of damage to existing grafts, ensuring their preservation during the transplant process.

How many grafts does someone need for hair transplantation to achieve the best results?
Please note that the number of needed grafts and the number of harvestable grafts are different. The number of needed grafts represents the minimum amount required to achieve a good optimal result after hair transplantation. On the other hand, the number of harvestable grafts depends on the donor area, which may be more or less than the needed grafts.
If you have a donor area with good density, the needed and harvestable number of grafts could be the same. However, if you have a large recipient area, the harvestable grafts may be less than the needed grafts. The surgeon must harvest a safe number of grafts to avoid harming the density or health of the donor area.
Why is there sometimes a significant difference between the suggested harvesting grafts among different clinics?
The significant difference in suggested harvesting grafts between different clinics can be attributed to various factors. Firstly, it’s important to note that the number of harvestable grafts is not necessarily indicative of a doctor’s experience or professionalism, nor does it determine the price of the procedure.
The variance in suggested graft numbers arises from the fact that there is a range of minimum safe and maximum risky numbers of grafts that can be harvested. Some clinics may provide patients with a logical and safe number of grafts, prioritizing the long-term health and density of the donor area. However, others may propose the maximum number of grafts to attract clients’ attention.
As mentioned previously, it’s crucial that the surgeon harvests a safe number of grafts to prevent damage to the density or health of the donor area. Therefore, it’s imperative to undergo a comprehensive consultation with a qualified hair transplant surgeon.
What are the risks of overharvesting grafts from the donor area during hair transplantation?
Overharvesting from the donor area poses several risks.
From a cosmetic standpoint, it can result in an unnatural appearance and thinning of the donor area.
However, the most critical concern lies in the medical issues it could cause. Overharvesting not only leads to a prolonged and extended recovery period and increased scarring but also carries a high risk of necrosis.
This means that overharvesting may disrupt the blood supply to the donor area, increasing the risk of tissue necrosis, which refers to the death of tissue cells.
Is it possible to harvest grafts from the chest and beard areas?
Yes, it’s possible to harvest grafts from the chest and beard for hair transplantation when donor hair on the scalp is limited. However, there are some disadvantages:
- Different hair texture.
- Increased risk of scarring and infection in the donor area.
- Potential follicle damage.
- Lower graft survival rate (30-40%).
Is hair transplantation painful?
Hair transplantation procedures are typically performed under local anesthesia, which numbs the scalp and minimizes discomfort during the procedure. While some patients may experience mild discomfort or pressure sensations during the process of graft extraction and implantation, the use of anesthesia helps alleviate any pain.
Additionally, many clinics offer options for additional sedation or relaxation techniques to further enhance comfort during the hair transplantation.
What does sedation mean during hair transplantation?
Sedation during hair transplantation refers to the use of medication to induce a state of relaxation and calmness in the patient. It helps alleviate anxiety and discomfort during the procedure, allowing the patient to remain comfortable throughout the process.
What is the difference between local anesthesia and sedation in hair transplantation?
The difference between local anesthesia and sedation in hair transplantation lies in their effects and administration. Local anesthesia is used to numb specific areas of the scalp where graft extraction and implantation will take place. It blocks the sensation of pain in those areas while allowing the patient to remain conscious and alert during the procedure.
On the other hand, sedation involves the administration of medication to induce a state of relaxation and reduced consciousness. It can range from mild sedation, where the patient remains conscious but feels more relaxed, to deeper sedation, where the patient may be less aware of their surroundings or even asleep during the procedure.
In summary, while local anesthesia blocks pain sensation in specific areas of the scalp, sedation aims to keep the patient calm and comfortable throughout the hair transplantation procedure.
Is sedation safe during hair transplantation? Can everyone use it?
Yes, sedation during hair transplantation is generally safe when administered by qualified medical professionals. However, not everyone may be suitable candidates for sedation.
Factors such as overall health, medical history, and individual response to sedative medications need to be considered before determining if sedation is appropriate for a patient.
Is there any restriction for individuals undergoing hair transplantation?
Yes, there are certain restrictions for individuals undergoing hair transplantation. These restrictions can vary depending on factors such as overall health, medical history, and specific conditions. Some common restrictions may include:
- Age: Hair transplantation is typically not recommended for individuals under a certain age, as the pattern of hair loss may not yet be stable.
- Scalp Conditions: Pre-existing scalp conditions such as dermatitis or psoriasis need to be treated and stabilized before undergoing hair transplantation.
- Health Conditions: Certain medical conditions such as uncontrolled diabetes, autoimmune diseases, bleeding disorders, and active infections may pose risks during the procedure and affect healing afterward.
If an individual suffers from any of the following diseases, hair transplantation is not recommended:
High blood pressure: Due to the injection of adrenaline during the procedure, there is a risk of heart attack.
Epilepsy: Hair transplantation is not advisable due to the risk of seizure during the procedure.
Heart disease: If the individual needs to take aspirin daily, it is not possible to undergo hair transplantation, as aspirin may need to be stopped for 4 to 7 days before hair transplantation to reduce bleeding risk.
Alopecia: If an individual suffers from alopecia, hair transplantation may not be effective.
Keloid: Individuals with a tendency to form keloid scars may not be suitable candidates for hair transplantation due to the risk of excessive scarring.
Cancer: If an individual has undergone chemotherapy their immune system is weakened.
Thyroid disorders: While hair transplantation is technically possible for individuals with thyroid disorders, it may not be advisable as they may continue to experience hair loss.
Diabetes: Only DHI (Direct Hair Implantation) may be possible for individuals with diabetes, as they may have increased risk factors for complications during other types of hair transplantation procedures.
Can people with sexually transmitted diseases (STDs) undergo hair transplantation?
Yes, People with sexually transmitted diseases (STDs) such as HIV and Hepatitis B and C can undergo hair transplantation. However, additional precautions are necessary, and standard equipment sterilization protocols are not sufficient. Since all equipment used in the procedure is typically single-use only, leading to extra charges for the procedure.
